Applying
Application Procedures
In
applying to our program we require that you submit the following materials:
- The Mount Sinai House Staff Application Form (which can be downloaded at
http://www.mssm.edu/fellowships/pdfs/fellapp.pdf).
- Two (2) letters of recommendation from people who have
known you in your professional capacity. By New York State law, all persons
hired must have in
their file a recommendation from their current employer. If circumstances
preclude
you from obtaining such a letter at this time, you must be prepared to
furnish one later as a condition of final acceptance into the program.
- A one or two-page statement of your long-term career goals
and your specific learning plan for the coming two years.
- If you are a
graduate of a foreign medical school, enclose a copy of your ECFMG
certificate. Also, indicate how many times you took the examination before
passing; we are required by New York State law to have this information.
- A current curriculum vitae.
All applicants are also encouraged to apply for the Occupational Physicians
Scholarship Fund; further information on the Scholarship can be found at www.opsf.org.
Completed applications are reviewed by a Departmental committee and selected
applicants are invited for interviews, which are scheduled at mutually convenient
dates. Most interviews are conducted between October and December. The Departmental
committee meets to rank the interviewed applicants after all interviews have
been completed. Applicants are notified on December 15, the Occupational
Medicine Residency notification date.
If you are accepted into the Residency Program you will be require to submit the following items prior to beginning:
A notarized copy of your M.D. Diploma or the original for us to view.
Your final medical school transcript with your school seal on it.
- Medical School Dean's letter.
- A copy of a valid CPR card.
- A copy of NYS license (needed by PGY 3 year).
- Two (2) letters of reference, one from prior training program.
- Certificate of Completion from PGY 1 and latest year of training completed (or an equivalent letter from your Program Director).
- Documentation of successful completion of your ACGME core competencies (from your Program Director).
