In addition, when you notify us that you have provided for Mount Sinai in your estate plan, you become a member of The 1852 Society of Mount Sinai. Here is sample bequest language: I hereby give to The Mount Sinai School of Medicine of New York University, a New York not-for-profit corporation located at One Gustave L. Levy Place, New York, NY 10029, the sum of $_______ [or % of the rest, residue and remainder of my estate] to be used for the benefit of The Zena and Michael A.
