Domestic violence should be considered in your differential. the most common cause for perforation of the TM is a slap to the ear or uncontrolled dive into a pool. This patient is crying and does not admit to any acute injury. A thorough histroy and physical should be performed, including documentation with photographs of any obvious bruises or soft tissue swelling or lacerations.
