In general, small meniscal tears not causing impingement of the joint can be managed conservatively (i.e., without surgery). However, the age of the patient and the level of physcial/athletic ability must be considered. Tears which are extensive and cause mechanical obstruction of the joint (e.g., a bucket handle tear which causes the joint to lock), should be repaired surgically. The outer 1/3 of the menisci are well-vascularized and will have some healing on their own. Also, in younger patients, tears which involve the vascular zone should be evaluated for repair, and not simple debridement, as this may significantly retard the progression of arthritis in the joint. If a meniscal tear is suspected, proper orthopaedic evaluation and follow-up is essential.
