Pediatric Interesting Case


Present Illness

A 10-month old girl presents with one day of mild URI symptoms and fever. She is cranky and is eating less, but still wetting her diapers. She had one episode of vomiting of food, but no diarrhea. She has an occasional cough and a runny nose. She has not traveled and has had no sick contacts.

Past History

She was full term, normal weight, and spontaneous delivery. She has no past medical problems and her immunizations are up to date.

Medications and Allergies

She takes no medicines and has no allergies.

Physical Exam

Appearance: VS Temp 104.1F, HR 146, RR 42, Sat 100%RA, Wt 9.1kg. She appears nontoxic but mildly irritable.
HEENT: NCAT AFOF She has clear rhinorrhea, a mildly injected throat, mild cervical adenopathy, and clear TM's.
Lungs: Clear, no rales, no retractions
Heart: good S1, S2 without murmurs
Abdomen: benign, no HSM
Extremities: 2+ pulses with good cap refill and no rashes
Neuro: Alert and active
