BeneFlex Program Summary

Disability Plan

• Short-Term Disability (STD) Coverage – You will be paid for any earned but unused sick time before you will receive payment for STD (see Sick Days and Medical Leave of Absence). However, you must complete an STD application (form DB450).for all periods of disability. The form and directions can be downloaded from:

  STD benefits are provided through UnumProvident and cover up to 26 weeks of disability.

• Long-Term Disability (LTD) Coverage – If you are certified by the insurance company as disabled beyond the 26-week Short Term Disability period, Mount Sinai provides LTD coverage through UnumProvident. Costs and benefits vary based on your salary, the benefit level and your age. See Medical Leave of Absence.
