BeneFlex Program Summary

Medical Plan

BeneFlex offers you a choice of two UnitedHealthcare PPO’s and three traditional HMO’s:

• UnitedHealthcare Plus (PPO)
• UnitedHealthcare Basic (PPO)
• HIP (HMO)
• AETNA (HMO)
• Blue Choice (HMO)

The UnitedHealthcare PPO’s offer affordable coverage and freedom of choice. Under these plans you can consult a physician of your choice, many of whom participate as Mount Sinai providers. You do not need to select a primary care physician and you can go to a specialist without a referral. If you use a UnitedHealthcare network provider, there will be modest office co-pay, but you will avoid deductibles and claim forms and preventive services will be covered. If you use an out-of-network provider, there will be deductibles and co-payments.
