Mount Sinai School of Medicine Research Application

The Mount Sinai Research Application is used for all research, fellowships, service, education, and training programs at MSSM. You should complete Institutional Review Board (IRB), Institutional Animal Care and Use Committee (IACUC), and Biosafety Office if relevant. These forms are available by following the Web addresses below.

Biosafety

Note: This form is submitted directly to the Biosafety Officer for Recombinant DNA Research and Animal and Human Subject Applications. This applies to new and competitive renewal applications. One submission per research protocol.

Submitting the Application

All applicable GCO, IRB, and IACUC forms should be submitted in one packet to the Grants and Contracts Office (Icahn Medical Institute, 4-79) for all new, competing, noncompetitive continuation, supplemental, and no cost-extension applications.

Intramural Submission (Mount Sinai School of Medicine and Affiliates)
Submit one original only of forms.

Extramural Submission (to outside agency)
Submit one original of MSSM forms plus 3 copies of the extramural application.

A fringe benefit rate of 26.5 percent must be applied to all federal agencies. A 27.5 percent rate must be applied to all non-federal agencies. These changes have been incorporated into the Budget Page (GCO Page Two) of the MSSM Research Application.
