Inpatient Consult Service

The Palliative Care Consult Team is available to all patients admitted to the Mount Sinai Medical Center. The interdisciplinary team consisting of physicians, nurses, social workers and massage therapists specializing in palliative care works with patients and their primary doctors to help develop a continuity plan for palliative care.

Reasons to request a palliative care consult:

  • Pain and symptom management
  • Counseling/guidance about care options in the face of serious illness
  • Changing goals of care
  • Assistance in developing a palliative care discharge plan
  • Transfer or admission to the Mount Sinai Palliative Care Unit for vigorous symptom management or upon transfer from intensive care units when goals of care are changing
  • Loss, grief, and bereavement support

Inpatient Palliative Care and Hospice Unit

The Mount Sinai Medical Center is currently in the process of establishing an Inpatient Unit specifically for palliative care and hospice patients.

Ambulatory Services

The Palliative Care Ambulatory Program is available on a consultation basis to ambulatory patients with serious, complex, or life-threatening illness. The Palliative Care Outpatient Program is staffed by physicians and nurses with specialized skills in palliative care and is located at Coffey Geriatrics Practice, at 1470 Madison Avenue in New York City. The service delivery model is predominately consultative with ability to provide co-management with select patients for whom the primary focus of care is palliative.
