Evaluation of Reaching Children Initiative is important to determine if this educational effort achieves its objectives. Input from clinicians is vital to the evaluation process. We ask physicians, regardless of whether they attended the CME activity, to take a few minutes to fill out a brief survey on their experience with mental health issues. Completed questionnaires can be faxed to Danielle Laraque, M.D. at (212) 241-8738.

At the completion of the program, clinicians who submitted a survey will receive the Reaching Children Initiative Toolkit CD-ROM with the materials developed for the program.
