If you are not in the metropolitan area of New York/New Jersey/Connecticut and cannot come to the Office of the Registrar to sign this Release Form, you may FAX it to our office at 212-369-6013 at your earliest. Failure to do so will jeopardize completion of your ERAS application and ultimately create problems with your interview schedule.
